fix: say the $10,000 fee-free tier is counted per program - #159

Code counts the free tier on each program's own total_revenue (affiliate-fee.ts:212), and Son decided 17/10 to keep it per program for now. Five hub and quickstart pages said "your first $10,000" with no scope; they now say each program's. Co-Authored-By: Claude Opus 5.5 (1M context) <noreply@anthropic.com>

The CMS counts the tier on the program's own
